Lost starboard airscrew 14/08/43 after skimming the water during a simulated torpedo run south of Wollongong NSW.
Crew: FLTSGT R.S.Roberts (pilot), PLTOFF Ronald Whitwell (observer), PLTOFF Neville Henrichsen and SGT Bruce Kent (WAGS) uninjured.
Damaged 13/01/44 when starboard undercarriage failed to come down on landing.
Crashed 19/07/45, Lettes Beach on a instrument flight from East Sale VIC.
Pilot FLGOFF Oswald "Ossie" Morgan and FLGOFF Archibald Bruce Hinge was the safety pilot.
Port engine caught fire, Prop was feathered and the fire extinguisher didn't work.
A wheels up landing was made near Lake Reeve at L bombing range the crew although badly burned survived.
